When services are purchased as a package or plan, Polk Funeral & Cremation Service can offer savings to the family. If deletions are made from any plan, the deductions will be made from the itemized total, not the plan total. These discounted packages are available only when a casket is selected at our funeral home. Casket prices are not included.

A. Complete Traditional Service

  • Basic services of funeral director and staff including death certificate service
  • Transfer of remains to funeral home
  • Embalming or other preparation of the remains
  • Use of equipment and staff for viewing/visitation (3 hours) (deduction fee if no viewing)
  • Set up procedures at church and staff for funeral rites
  • Register book, Pallbearer Gloves, acknowledgment cards
  • Funeral coach OR transportation to the crematory

B. gRAVESIDE OR nICHESIDE sERVICES

  • Basic services of funeral director and staff including death certificate service
  • Transfer of remains to funeral home
  • Embalming, Refrigeration, or Cremation
  • Use of the equipment and staff for visitation/viewing (3 hours)
  • Staff provided for direction of graveside service
  • Register book, Pallbearer Gloves, acknowledgment cards
  • Funeral coach

C. iMMEDIATE bURIAL

  • Basic services of funeral director and staff including death certificate service
  • Transfer of remains to funeral home
  • Sanitation, wrapping and casketing of unembalmed remains
  • Transfer to cemetery (at non-specified time)

D. fORWARDING rEMAINS TO ANOTHER moRTUARY

  • Basic services of funeral director and staff including death certificate service
  • Transfer of remains to funeral home
  • Embalming (Autopsy/Tissue Harvest Case additional fees)
  • Local Transportation to Common Carrier

E. rECEIVING rEMAINS FROM ANOTHER mORTUARY

  • Basic services of funeral director and staff
  • Local Transportation from Common Carrier
  • Funeral coach to cemetery with direction
  • Refiling of disposition permit at local health department


F. Traditional Service prior to Cremation

  • Basic services of funeral director and staff including death certificate service
  • Transfer of remains to funeral home
  • Embalming or other preparation of the remains
  • Use of equipment and staff for viewing/visitation (deduct fee for no viewing)
  • Use of equipment and staff for funeral ceremony or staff to direct service at other facility
  • Register book, Pallbearer Gloves, acknowledgment cards
  • Funeral coach for Service
  • Transfer to crematory
  • Cremation process. if decedent is not under 250lbs, additional fees may be required for a suitable container.

G. Cremation Followed By Memorial Service

  • Basic services of funeral director and staff including death certificate service
  • Transfer of remains to funeral home
  • Minimum preparation, casketing and refrigeration of unembalmed remains
  • Use of equipment and staff for memorial ceremony
  • Register book, Pallbearer Gloves, acknowledgment cards
  • Transfer to crematory
  • Cremation process. if decedent is not under 250lbs, additional fees may be required for a suitable container.

H. Viewing/Visitation prior to Cremation (no rites)

  • Basic services of funeral director and staff including death certificate service
  • Transfer of remains to funeral home & Transfer to Crematory
  • Minimum preparation of the remains (Our policy is not to dress unembalmed remains.)
  • Use of crematory facilities and staff for viewing/visitation (1 hour)
  • Cremation process. if decedent is not under 250lbs, additional fees may be required for suitable container. (includes alternative container and temporary plastic urn)

Direct Cremation

A “Direct Cremation” is a disposition of human remains by cremation without viewing, visitation or rites or ceremony.

  • Basic services of funeral director and staff including death certificate service
  • Transfer of remains to funeral home & Delivery to Crematory
  • Minimum preparation and refrigeration of unembalmed remains
  • Cremation process. if decedent is not under 250lbs, additional fees may be required for suitable container.

If you want to arrange a direct cremation, you can use an alternative container. Alternative containers encase the body and can be made of materials like fiberboard or composition materials (with or without outside covering). The containers we provide are made of heavy duty fiberboard suitable for cremation.
